Adonis "Donnie" Creed, then a young man, sneaks out of Los Angeles in 2002 to watch his best friend, Golden Gloves champion Damian "Diamond Dame" Anderson, compete in an underground boxing match. He tells Donnie after Dame's victory that he wants to become a professional wrestler and win the world championship. Donnie attacks Leon impulsively during a detour at a liquor store.
Donnie prevails over newly released "Pretty" Ricky Conlan in a 2017 rematch. Donnie gives up boxing to spend more time with his wife Bianca and their daughter Amara, who was born deaf, and learns American Sign Language well. Donnie runs Delphi Boxing Academy with Tony "Little Duke" Evers Jr. three years later, in 2020, and he is promoting his protégé, world champion Felix "El Guerrero" Chavez, for a fight with Viktor Drago. Bianca gives up performing and becomes a successful producer because of her hearing loss. Amara aspires to become a boxer like Donnie, which gets her in trouble at school for punching another student, while the two watch Mary-Anne, Donnie's adoptive mother, get worse.
After Dame is released from jail, he reconnects with Donnie, with whom he discusses his desire to return to boxing. Donnie reluctantly invites Dame to the gym to spar with Chavez, but Duke and Chavez despise Donnie's aggressive style. Later, Dame goes to Donnie's house, where he meets his family and tells Bianca about their time in a group home, which Bianca had never heard of. Dame requests a title match against Chavez in private, citing Donnie's previous one-in-a-million shot, which Donnie considers impossible.
At a party for Bianca's record name, Lady mysteriously informs Bianca regarding Leon and that Donnie was keeping it mysterious from her. Drago suffers an unidentified attack and is unable to compete against Chavez because of his injuries. Donnie suggests Dame as an opponent, pitching an underdog fight similar to Rocky Balboa's first title shot, which Chavez accepts because he does not want to lose the money that was spent on the event. Dame fights dirty during the fight, but she defeats Chavez and takes the undisputed heavyweight championship.
Donnie goes to Mary-Anne after the match, feeling uneasy, and she shows him letters that Dame wrote to Donnie while she was in prison but kept from him because she thought he was a bad influence. A picture of Dame with a fellow inmate that Donnie recognizes as Drago's attacker is included in one letter. Donnie confronts him, realizing that Dame orchestrated the attack. Before giving him a black eye, Dame reveals that he had manipulated all of their encounters to position himself as a champion. Donnie is unable to discuss his guilt with Bianca because Dame enjoys his newfound fame and publicly denigrates him as a con artist who abandoned him.
After having a conversation with Donnie, Mary-Anne believes that he is Apollo, but she suffers another stroke and passes away in his arms. Donnie tells Bianca after her funeral that Leon was the abusive caregiver in the group home where he and Dame lived prior to his adoption. Dame pulled a gun when Leon's friends got into a fight after Donnie attacked him at the liquor store. Donnie fled when the police arrived, but Dame was taken into custody. Donnie never contacted Dame out of shame and guilt, trying to forget the memory. He tells Bianca that there is only one way to bring Dame down, knowing that she will not stop defaming him.
Donnie appears on First Take, where Dame calls in to persuade him. Donnie declares his retirement and accepts Dame's challenge for the championship. Donnie will compete against Dame in the "Battle of Los Angeles" at Dodger Stadium following training with Duke and a recovered Drago. The match between Donnie and Dame is exhausting and evenly matched. Donnie has dreams of his oppressive cultivate home and Woman's life in prison, prompting a knockdown in the last round. Donnie defeats Dame to regain the championship and win the match after letting go of his fear and guilt when he sees his family in the crowd. Donnie and Dame reconcile afterward, and both men acknowledge that neither of them was to blame. Donnie pretends to box with Amara as he joins Bianca and Amara in the empty stadium's ring.
